Call for examples of good practice
Introduction
The Department of Health is today publishing a call for examples of good practice from services, which can demonstrate that they are:
• supporting implementation of the NSF for Long-term (Neurological) Conditions; and,
• making a positive difference to people living with long-term neurological conditions.
Areas of interest
We are interested in examples of good practice in the following areas:
1. Local and regional implementation arrangements, including development of joint teams, collaborative commissioning, leadership arrangements and neuroscience networks.
2. Integrated health and social care assessment and care planning for people with long-term neurological conditions.
3. Meeting the information needs of people with long-term neurological conditions, e.g. information about local services, neurological conditions, information and support to self-care such as expert patient programmes.
4. Development of specialist practitioner roles in neurology and evidence of their impact.
5. Development of care pathways for common neurological conditions.
6. Improving knowledge and experience of neurological conditions in care providers, e.g. personal care assistants, agency staff and staff in acute trusts.
